Welcome to the webpage for parents/guardians whose children have been selected to participate in the StFX-CMS Math Camps. The organizers are committed to providing your children with an excellent math camp experience. We also want your children to have fun at the Camp. We need your help in hosting a successful camp, and request that you kindly review the material below and take all required actions. You will need to submit the following two documents by Monday April 9, 2018:
- 1. Informed consent, risk acknowledgement, and indemnity agreement (submit by regular mail, fax or e-mail).
- 2. Personal information form (submit online).
The activities of the camp (e.g., listening to presentations by professors, participation in math trivia, math relays, problem solving) are very low risk activities. However, it is required by The Governors of St. Francis Xavier University that an INFORMED CONSENT, RISK ACKNOWLEDGEMENT AND INDEMNITY AGREEMENT be signed by parents/guardians whose children plan to participate in the Camp. Please click here to view the agreement. We request that you print and complete the form if you agree to the terms of the agreement. The completed form must be mailed or faxed or e-mailed to the address or fax number or e-mail address shown on page 2 of the agreement form.
It is important that the camp organizers have some information about your child to better manage any situations that may arise. It is requested that you supply information (e.g., contact information for parents/guardians, allergies). In the next paragraph, you will be presented with an online version of the form for completion.
It is important to complete and submit the form supplying information about your child in one sitting as you will not be able to save the form and return to it later. To prepare for the Camp, please make note of the following: bedding, towels and toiletries are all included with the dorm room; note pads, clipboards, pens will be provided; students should bring scientific calculators.

We ask that your child arrive between 9:15 am and 9:45 am on Friday May 18, 2018 at MacIsaac Hall at St. Francis Xavier University in Antigonish. The address is 2200 Varsity Drive and a map of the StFX campus can be viewed by clicking here.
We ask that your child depart from MacIsaac Hall at St. Francis Xavier University in Antigonish between 11:30 am and 12:30 pm on Sunday May 20, 2018. The address is 2200 Varsity Drive and a map of the StFX campus can be viewed by clicking here.
