STEP 1: Recommendation Form (Closing Date is Friday March 8)

Please note that there are two parts to the form. Part 1 (pages 1 to 2) of the form requires student/parent or guardian/doctor/other emergency contact information. It is intendend that the parent or guardian will provide this information to the school teacher completing the recommendation form. This information will help the organizers and chaperones to better manage any situations that may arise. Part 2 (pages 4-5) of the form requires school and academic information on the student being nominated.
  • Complete the recommendations(s), one per student, to a maximum of two students per school. Ensure you have the required information as described in preceding paragraph before proceeding. You cannot return to the online form once it is closed and so recommendation of a student must be submitted in one sitting. Click here to gain access to the online recommendation form.
